You MUST have the following information available for each family member before you begin the application process:
- Last name, first name, middle initial
- Mailing address
- Phone number
- Social Security number
- Date of birth
- Income information for each family member
ASSISTANCE FOR ONLINE APPLICATIONS IS AVAILABLE FOR ELDERLY AND PERSONS WITH SPECIAL NEEDS BY CALLING THE APPLICATION HOTLINE AT 228-248-1030.
If you do not have access to a computer, they are available at your local public library and at many local social service agencies. (Paper applications will not be available)
